instances an applicant’s parent(s) birth
certificate maybe required
3. Four (4) very current colored Passport size
photographs of the applicant on plain background
with full face and both ears visible
4. Copy of valid Residency permit.
5. Copy of Kenyan ID.
2. New Passport (Minor)
1. Four (4) current colored Passport size
photographs of the child with full face and both
ears visible and with plain background
2. Passport copies of both parents
3. Copy of parent’s national identity card.
4. Child’s original birth certificate. If not in
English, it must be translated at the
Embassy.Note: Birth Notification certificate and
Hospital Entry forms are not accepted
5. Copy of parents residency permits

3. Replacement of a Mutilated Passport
1. Mutilated Passport to be replaced and its
copy
2. Copy of national identity card.
3. An affidavit detailing the circumstances
under which the mutilation occurred
4. Four (4) very recent colored Passport size
photographs of the applicant on plain background
with full face and both ears visible
4. Replacement of a Lost Passport
1. Two (2) duly completed Passport
